Rebeca Andrade, who replaced American Jade Currie in the competition record after she failed to reach the final in Paris, scored 14.166 points, beating Biles, who scored 14.133, after two retirements.
North American Jordan Chiles, making her Olympic apparatus final debut, scored 13.766 to win bronze.
Rio 2016 floor champion Simone Biles, who had just finished fifth on balance beam in an event won by Italy’s Alisa D’Amato, left Paris with three gold medals – team, all-around and vault – and a silver.
Rebeca Andrade added Monday’s gold to her silvers in the all-around and vault, as well as her team bronze, the first Olympic medal for a Brazilian gymnastics team.
