A 73-year-old woman died after hospital staff mistook her for a 90-year-old patient who had a “do not resuscitate” warning on his medical record in England, according to a report. Daily mail. Pat Dawson, a retired nurse, was otherwise healthy and was admitted to hospital with a suspected bowel obstruction.
The woman was taken by ambulance to Royal Blackburn Hospital and died after she collapsed and staff stopped trying to resuscitate her. When doctors realized that the patients’ records were mixed up, it was no longer possible to save the elderly woman.
According to Daily mailPat Dawson’s NHS number, gender and age have not been confirmed.
The hospital’s head of emergency services said the tragedy was an “honest mistake as staff were in a hurry” as the department was “overwhelmed and overcrowded”.
“Given the ongoing pressure on emergency departments, investigators are concerned about the possibility of a similar event occurring in the future,” said the person in charge of the investigation, quoted by the publication. Daily mail.
A post-mortem computed tomography scan showed that the woman’s cause of death was aspiration pneumonia, which occurred after inhalation of stomach contents.
