According to Reuters, the accident occurred when the Visakhapatnam-Rayagada passenger train stopped this Sunday due to a broken overhead cable and the Visakhapatnam-Palasa Express collided with the stopped train, derailing two coaches.
A preliminary investigation concluded that “human error” led to the Visakhapatnam-Rayagada train “overriding the signal” to stop, the Ministry of Railways concluded.
